2021-22 REPORT CARD

All athletes in GRADES 3-8 are rquired to submit the complete AYF Packet, Birth Certificate and Report Card.  Please go on Genesis and print it out now as Genesis shuts down in the summer and access to report cards might be increasingly difficult. No athlete will be permitted to practice in August without completed forms.

Please note that the Medical Clearance form & HJR Medical form must be signed by a doctor.  Medical forms expire one year after the exam date.

Completed forms may be emailed to medical@jrraiders.org with your child's name in the subject line or brought to the July camp.

There should be 10 PAGES in total- 8 in the Packet - Birth Certificate - Report Card  

 No athlete will be permitted to practice without completed forms.
