Medical Form Management

We are putting a new medical form tracking system in place and we need everyone's help!

We need medical forms from ALL players. This includes girls who played field hockey in the fall. These forms CAN be a copy and do not need to be the original!

You may either bring your medical form to your coach or you may hand it in/email it to Kristin Redziniak who will in charge of all medical forms for Girls Field Hockey & Girls Lax moving forward. Kristin's email is: kspedteach@aol.com
